I understand that the products and services offered by RAYS OF LIGHT LI LLC support the body, mind, and soul’s natural ability to create wellness in my life. I understand that RAYS OF LIGHT LI LLC owners, agents, staff and/or practitioners do not diagnose conditions, nor do they perform medical treatment, prescribe substances or medication, nor interfere with the treatment of a licensed medical professional. I understand that energy healing, or any service provided by RAYS OF LIGHT LI LLC does not take the place of medical care. It is recommended that I see a licensed physician or licensed health care professional for any physiological or psychological ailment I may have. I understand that energy healing provided by RAYS OF LIGHT LI LLC can complement any medical or psychological care I may be receiving, but not supplement it. I also understand that the body has the ability to heal itself if put in the right environment and in order to do so, complete relaxation is often beneficial. I acknowledge that long term imbalances in the body may require multiple sessions in order to achieve the level of relaxation the body needs for recovery and restoration. RAYS OF LIGHT LI LLC is not liable for my medical, mental, or emotional conditions before, during, or following any service, lecture, meditation, class or event provided by RAYS OF LIGHT LI LLC.

I take personal responsibility for my well-being and with respect for myself, I gratefully accept control of my choices. My heirs, guardians, legal representatives, and I hereby and forever indemnify, defend and hold harmless RAYS OF LIGHT LI LLC, and/or any of their owners, managers, agents, officers, employees, associates or affiliates from and against any and all actions, claims, suits, liabilities, damages, losses, expenses and costs, including reasonable fees, arising out of or resulting from my participation and experience with any of RAYS OF LIGHT LI LLC’s services, classes, meditations and/or events. I take full responsibility for my choices and am solely responsible for all liability for loss or injury incurred while in association with RAYS OF LIGHT LI LLC and/or any of their owners, managers, agents, officers, employees, associates or affiliates.

In the case of my attendance or participation at any lecture, meditation, class and/or event hosted by RAYS OF LIGHT LI LLC, I hereby consent to RAYS OF LIGHT LI LLC’s use of any photographs and/or video recordings taken of me, without compensation, to be used for any and all promotional or advertising material produced, marketed and/or published by RAYS OF LIGHT LI LLC.

I have carefully read this agreement and fully understand its content. I am aware that this is a waiver and release of potential liability and a contract between the above noted parties and myself. I understand that this contract is binding and acknowledge that I am signing it with my own free will, without coercion or duress.
